Case (43)
« on: December 16, 2008, 07:26:29 PM »
40yrs old women come to our hospital with epigastric pain, nausea, vomiting, and no stool passed nor flatus for 5 to 7 days and also she told me that she has urgency, burning of urine.
on physical examination her blood pressure was 120/60 and temperature 37degree centigrade, pulse 80 bpm.
1.what do you suspect?
2.what type of laboratory test is appropraite to send?
3.what is the differential diagnosis?
4.what type of treatment you give this patient?
the pateint now is in our ward , her condition is improving now, no burning sensation on urination, no abdominal pain, no nausea.

suspected diagnosis is intestinal obstruction
lab request
barium enema
Abdominal CT scan
Upper GI and small bowel series
Abdominal film
treatment
The objective of treatment is to decompress the intestine with suction, using a nasogastric (NG) tube inserted into the stomach or intestine. This will relieve abdominal distention and vomiting.

Surgery to relieve the obstruction may be necessary if decompression by NG tube does not relieve the symptoms, or if tissue death is suspected.
