Position Description

Bi-Lingual Case Manager (Somali-English)
Description: Mental Health Resources, Inc. (MHR), a progressive community mental health agency providing services throughout the metro area, has a full-time position available providing Rule 79 Case Management services to adults in Hennepin County who are experiencing severe and persistent mental illness.
Duties: Duties and Responsibilities include:
•Screen and assess short-term needs to adults with mental illness
•Assist with clients in accessing mental health, medical, social, education and vocational services
•Develop personal mental health treatment goals and care plans
•Coordinate resources and referrals for needed services
•Assist with civil commitment matters
•Provide outreach to clients in the community

Requirements:
•Qualified candidates for the position will possess a minimum of a bachelor’s degree in social work, psychology or another human service related field.
•A minimum of 2,000 hours of experience providing services to people diagnosed with severe and persistent mental illness and under the supervision of a mental health professional is required.
•Competencies at effective engagement skills, ability to assess change in function and mental status, organization skills and attention to detail, and teamwork.
