Description:
The City of Seattle Human Services Department is seeking a Somali, Russian, or Punjabi Speaking Case Manager to join the Aging & Disability Services Division. This an exciting opportunity to be part of a department dedicated to connecting people in the Seattle community with resources and solutions during times of need, and improve the health and quality of life for seniors and adults with disabilities.
The Case Manager will conduct in-home comprehensive functional assessments for frail elderly and disabled adult clients receiving DSHS Medicaid funded personal care and other services. Case Managers develop an individualized service plan based on the functional assessment with the client that identifies priorities, desired outcomes, and the strategies and resources to be used in attaining the outcomes. Reporting to a Human Services Supervisor, the Case Manager will coordinate and monitor 80-100 client care plans.

Additional Qualifications:
Required Qualifications:
Bachelor's degree in Social Services, Psychology or a related field (or a combination of education and/or training and/or work experience which provides the ability to perform the work of the class)
Three years social services experience involving interviewing, counseling, or crisis intervention
Preferred Qualifications:
Second language fluency in Russian/Ukraine, Somali, Spanish and/or Punjabi with the ability to effectively communicate verbally and in writing in English and second language
Experience working in major social service, health and/or government agencies
Experience working with elderly or disabled adults who have unstable health conditions and risk factors including multiple physical and/or mental health problems, long-term care needs, and/or drug/alcohol problems
Experience conducting comprehensive medical/psychosocial assessments while administering and monitoring service delivery
