Theoratically I can explain such a condition in this way:

I think this man's ears function normally but he does'nt have external ear (Congenital atresia), and this condition always associated with middle ear abnormalities so in some way he may have open connection b/w middle ear and mouth (anatomically eustachian tube connects both) or his eustachian tube is only obstructed and normally when you yawn or open your mouth widely it relieves!!!
