Salam to All,
A week ago in my PHC clinic presented with father, a 9 yrs old boy, in good general condition, after the usual introductory routine, I asked the father what was the reason of the visit, He reply that his son had the last 2 to 3 days mild abdominal pain and distention, and no stool passed for the last 5 to 7 days, when I asked if it is the first time the boy had this problem, he reply, not at all, and informed that there was several such problem for the last year and in one occasion the boy was kept in hospital several hours.
On examination the patient 9 yrs old boy, with fairly pale mucous membranes and moderate abdominal distention, heart and lungs normal with mild innocent heart murmur, on palpation of the abdomen was fund multiple hard masses of different sizes ans shapes.
What would be your provisional diagnosis?
And what will be your next course of action.
The boy was seen in my clinic after 5 days and was well improved, no abdominal pain, no distension and with soft and lax abdomen, all masses disappeared.

i think its functional abdominal pain (Irritable Bowel Syndrome)
And what will be your next course of action.
A comprehensive history, a physical examination, laboratory, microbiology and radiographic studies, to exclude an organic diseases.
Management of this patient will include:
# Reassure the patient that the absence of an organic pathology indicates a normal life expectancy.
# Emphasize the expected chronicity of symptoms with periodic exacerbations.
# Teach the patient to acknowledge stressors and to use avoidance techniques.
" Fiber supplementation and laxatives
Antispasmodic.

DEFFERENTIAL DIAGNOSIS:
will be bowel obstruction,prolonged constibation, peritonitis or diverticulitis
NEXT COARSE OF TRETMENT:
history physical examination laboratory endoscopic histologic x_ray of abdomen
usufull tretment will be Laxatives its products that promote bowel movements.include inSaline cathartics dibasic sodium phosphate (Phospo-Soda), magnesium citrate, magnesium hydroxide (milk of magnesia), magnesium sulfate (Epsom salts), sodium biphosphate, and othersclude
