Title: Cousin couple.. Is there baby at risk?

Is it totally 100 percent birth defect for their baby? or..

it is not 100% percent , even if you are first degree cousins, this depends on the presence of previous Genetic diseases or Malformations in the family. it is difficult to develop to develop new genetic diseases, and even if there is, it needs mutation (e.g radiation exposure--which is not related to consanguineous marriage).
There are specific Genetic diseases that both of you may have some small share of it but your combination may lead to full blown disease (e.g Fragile X syndrome).

Genetic Consult can be done and is strongly recommended in  positive family history of hereditary diseases. Anyway you should do it just to be on the safe side.....
