SOS CV International has been running its operations in Somalia since 1985 when the first SOS Children's Village was opened in Mogadishu. Despite the ever changing political and social instability and constant high level of insecurity, SOS CV International has maintained continuous presence in the country in general and in Mogadishu in particular. Programs/projects were later expanded in Somaliland (North-West Somalia) that included a Children's Village, Medical Centre, Schools and Family Strengthening Program. In South Central Somalia, where the humanitarian situation is in crisis, SOS CV International is implementing Health, Nutrition and Emergency Relief Programs in Mogadishu and Baidoa districts. Target beneficiaries for health services are community members, especially children under five years and pregnant women and lactating mothers from socially and economically marginalized backgrounds who cannot afford health care provided elsewhere. SOS services which include health and nutrition care are delivered free of charge.

The nutrition programme targets severely malnourished children 6-59 months of age and pregnant and lactating women. Severe acute malnutrition is diagnosed according to international and Somali Nutrition Cluster agreed criteria. Pregnant women are eligible from their 2nd trimester, and lactating women if they have a breastfeeding infant below 6 months of age. The supplementary feeding programme targets moderately malnourished children.
The principal objective of the programme is “to improve health care for women and children in Somalia”.

The specific objective is “improved basic health care services, nutrition and maternal and neonatal health services for vulnerable women and children”.

BACKGROUND TO THE ASSIGNMENT
Obstetric and neonatal care is one of the core businesses of SOS Mother and Child Hospital in Mogadishu. SOS Mother and Child Hospital is a 114 bed capacity hospital that provides care to both mother and child. About 450 deliveries are conducted monthly; of this 50 are caesarean sections.

Somalia has amongst the highest maternal mortality rates in the world, estimated at 1000/100,000 live births. At the SOS hospital the maternal mortality has been estimated to be 0.6%, though this has been noted to have increased significantly in the last few months.

The obstetric services offered at the hospital include: antenatal ward, delivery unit, operation theatre for caesarean sections, postnatal ward, maternity outpatient, antenatal and postnatal clinics. The common maternal complications seen include: antepartum hemorrhage, postpartum hemorrhage, eclampsia, pre-eclampsia, obstructed labour and anemia.

The obstetric services are in need of improvement and refreshing of the staffs’ knowledge in current obstetric services

Through this consultancy assignment SOS would like an assessment carried out to identify the gaps in the obstetric services currently provided and an on-the-job training carried out that covers the main conditions seen in emergency obstetric care. These conditions include but not limited to: antepartum hemorrhage (with special emphasis to abortions and post-abortal care), postpartum hemorrhage, heart disease, diabetes, uterine rupture, infections (pre & post-delivery), eclampsia, pre-eclampsia, obstructed labour, anemia, coma in pregnancy, candidiasis and UTIs.

The training should also comprehensively cover triage in an obstetric department, identification and management of normal labour and abnormal labour and assess and correct any gaps in the performance of caesarean sections and post-operative care.

OBJECTIVES
The overall objective of the proposed consultancy is to improve emergency obstetric and neonatal care in the hospital. The specific objectives are:

3.1 To carry out a training in emergency obstetric and neonatal care

3.2 To carry out training in clinical management of pregnancy related medical conditions

3.3 To establish a triaging system for the management of obstetric patients

3.3 Write a training report clearly outlining the topics covered, training methodology and techniques employed, the level of participants’ involvement and their individual action plans on how they will use the new knowledge and skills acquired.

METHODOLOGY
The Consultant will be expected to employ a blend of participatory methods of training, training delivery will include but not limited to adult learning principles, lectures, group work, open discussion, practical demonstrations and do-it-yourself sessions.

SCOPE OF WORK
The assignment will involve all the staff offering obstetric services at the hospital. The consultant is expected to among other tasks:

5.1 Carry out training in emergency obstetric and neonatal care

5.2 Carry out training in clinical management of pregnancy related medical conditions

5.3 Establish a triaging system for management of obstetric patients

5.3 Produce a comprehensive training report.

TIME FRAME
The assignment will be undertaken within 11 days (inclusive of 2 travel days), from 28th January to 7th February 2014, broken down as follows:

6.1 Travel to Mogadishu – 1 day.

6.2 Conduct training needs assessment – 1 day

6.3 Training on emergency obstetric care – 5 days.

6.4 Training on clinical management of pregnancy related medical conditions – 2 days.

6.5 Establishing a triage system for obstetric patients – 1 day

6.6 Travel out of Mogadishu – 1 day.

DELIVERABLES
The Consultant is expected to deliver the following outputs for this assignment:

7.1 A 5 day training on emergency obstetric care delivered to 20 participants

7.2 A 2 day training on clinical management of pregnancy related medical conditions delivered to 20 participants

7.2 Triage system established

7.2 A comprehensive report for the assignment produced as per the TOR.

QUALIFICATIONS AND RELEVANT EXPERIENCE.
Applicants must have:
• University under-graduate degree in medicine and post-graduate in obstetrics & gynecology.
• Minimum of 5 years’ experience in health work.
• Experience in emergency obstetric care.
• Experience with emergency health and nutrition response programming including emerging response strategies.
• Demonstrated understanding of hospital set-up and management.
• Excellent documentation and communication skills.
