Medecins Sans Frontières Intersectional Regional Pharmacist based in Nairobi / Kenya

Background MSF has a strict policy concerning procurement of pharmaceuticals, intended to avoid potential problems with the quality of the medical supplies in the countries where we work. Risk factors specifically linked to local procurement of drugs include: weak governmental control on manufacturing, distribution and import; inappropriate quality standards; illicit distribution channels; and presence of fake products. Therefore, general first-line procurement strategy for pharmaceuticals is international supply from MSF Procurement Centres. If purchases of pharmaceuticals are necessary in a given country, for instance because of legal constraints, and have been approved by the Medical Director of the concerned section, a technical visit will be done by one of the section pharmacists, to identify local suppliers of acceptable quality. Then, a competent person will be identified within each mission to be responsible for local purchase according to the indications of the section pharmacist; this person can be an expatriate or national pharmacist or nurse, and is in charge of the daily management of local purchase (stock planning and supervision, orders at approved suppliers, follow-up of suppliers, etc). It is stressed that the decision to approve a local supplier is to be taken by a section pharmacist, according to the principle of shared decision at HQ level. The regional pharmacist position in Nairobi was created to ensure that all sections purchasing pharmaceuticals in Nairobi follow the same principles and policies for local purchase, in close link with the international pharmacists’ pool. With the mandate from the medical directors, the regional pharmacist position in Nairobi was established in March, 2005. Evaluations of the position took place in 2006, 2007 and 2008. At present there is a commitment to maintain the position. The general objective of the position is to support and supervise MSF programs and sections in pharmaceutical supply, by guaranteeing that MSF quality standards are applied according to the evolving situation in each country and available updated information.

Place within the organisation •Hierarchically accountable to the International Medical Coordinator and technically linked to the International Pharmacist Coordinator; while being in the field and under administrative and security management of MSF-Spain /OCBA through MSF OCBA mission in Kenya". •This intersectional regional pharmacist position will be functionally linked to the international pharmaceutical coordination: it will be responsibility of the international pharmacist coordinator to ensure that the regional pharmacists is given appropriate technical back-up, by: ?- considering him/her as a member of the international pool (including two participations per year in international pharmacists meetings); ?- identifying a regular technical back-up among the section pharmacists - continuously sharing with him/her relevant updated information on sources and qualities; ?Providing him/her with any needed information on demand. Ensuring the links with the pharmacists in the Campaign team and in Procurement Centres.

Main purpose of the job •To ensure that the proposed procurement strategies, as approved by Medical Department in the HQ (of each project’s section), are followed and implemented by the teams in the region. •To monitor and re-evaluate the relevance of the procurement strategies in a continuous way, taking into account MSF program developments in the countries, quality, legal and administrative issues. •To document the quality of the locally available products, and make recommendations to the sections. Decisions on the acceptance or rejection of a source will be under the responsibility of the international pharmaceutical coordinator. •To ensure continuous follow-up of the quality of local suppliers. The position will cover the following region: Kenya, South Sudan, Somalia and Uganda and only exceptionally (with Medical Director’s approval) the surrounding countries.

Core responsibilities and tasks

Context information The technical, qualitative, political and legal description of the pharmaceutical environment will be updated as necessary:

Update description of current needs and supply strategies in MSF programs
Update on legislative aspects of purchasing and importing pharmaceutical products as NGO, including products under International Control and patented products,
Update on changes in pharmaceutical legislation and quality control performed by the DRA of the concerned countries
Pharmaceuticals procurement ? Follow up and monitoring of identified sources for medicines for MSF projects. Identification of new sources will be done according to quality standards defined by the international pharmaceutical coordination, to feasibility (and sustainability if needed), price and legal/administrative constraints: -MSF supply centres in Europe -Local manufacturers -Local wholesalers, both private and not-for-profit. ? To be the main referent for CHMP for all procurement related issues and ensure that CHMP complies with MSF requirement for local procurement. ? To establish and continuously update a list of selected suppliers per product (procurement databases , to be sent to section pharmacists for validation), to be fed by information collected locally and by information provided by the international pharmacists’ pool; to ensure continuous quality monitoring; to compile and up-date documentation for each product (selected supplier, qualitative and legal dossier, prices, quality problems); to manage any eventual problems with the suppliers and/or the products (in coordination with project teams). ? Send three times a year a bullet point report on emergency pharmaceutical orders purchased in Nairobi for neighboring countries ? Update the international pharmacists’ coordinator and section pharmacists with new findings
Support to MSF supply centers in Nairobi The regional pharmacist should monitor and share with international pharma coordinator the developments and ensure that all pharmaceutical activities of the MSF supply centres present in Nairobi (KSU, Nairobi Supply Center, SUKA) remain within the frame of the international medical procurement policy. The regional pharmacist may also provide technical support where relevant to the structures. The regional pharmacist should support and train the pharmacists when necessary.
Technical back-up to MSF teams ? To be the technical back-up for all pharmacy related questions of all the medical coordinators, medical purchasers and any pharmacists (both national and expatriates staff) working in capital offices and/or in the field.
??To provide briefing to all medical coordinators, pharmacists and any persons concerned with medical supply.

? To help with the organisation of regional pharmaceutical trainings in case is needed.

? The need for field visit, pharmaceutical assessment of the project, or specific training should be discussed with section pharmacists and International Pharmacist Coordinator (IPC), who will send the request to the regional pharmacist.

External representation To be the MSF focal point for the national Drug Regulatory Authority, in collaboration and in support to the Pharmacists and Medical Coordinators.
To act as a liaison person on pharmaceutical issues with relevant MoH departments, other NGOs etc as necessary and in agreement and collaboration with the medical coordinators.

Access to new treatments If needed, to support with technical pharmaceutical expertise the Medical Coordinators and Head of Missions, in their activities to improve access to treatment.
Pharmacovigilance Be the focal point for complaints related to medical items use, and give support for batch recall processes under the coordination of the IPC.

Candidate profile
• Degree in Pharmacy (or equivalent diploma according to the country of origin of the pharmacist) • Experience with quality assurance and pharmaceutical procurement is desirable • Pharmacist with field experience in developing countries (MSF experience is an asset) • Fluency in English (written and spoken), knowledge of additional languages of the region are desirable • Ability to work in a team • Good organisational skills • Good communication skills, notably the capacity to negotiate and gather information • Ability to represent MSF at external meetings

Salary & Working conditions

• Contract: expatriate status, with a salary level equivalent to a medical coordinator field position. • Duration: 1 year, and possible extension after evaluation and decision by the medical directors. • Location: Nairobi, Kenya. The person will be based in MSF-Spain office in Nairobi providing the direct administrative support. • Starting date: 1st of February 2012
