Mr Abdi Husein is a 55-year-old Library manager.
He has been admitted to hospital for investigation of increasing cardiac failure.
Examination reveals that he has an enlarged liver.
It is noticed that his skin is a uniform dark colour.
Also there is abnormal glucose tolerance as revealed by the investigation.

Questions:

1 What is the diagnosis?

2 What pathology occurs in the liver?

3 What is the reason for abnormal glucose tolerance?

4 After correctly diagnosing and treating Mr abdi, what else must be done?

The 15 point of this case goes to Ladan, who answered it correctly and this was her answer:

1) Hemochromatosis

2) Iron deposition in liver cells

3) Iron deposits in Pancreas, hence the cause of DM (Bronze diabetes).

4) Diagnosis is liver biopsy and treatment is phlebotomy. The other thing that should be done is genetic studies for himself (whether he is homozygous or heterozygous carrier) and immediate family.

PS: Iron deposits in the heart causing Diastolic heart failure.
